La intolerancia a la lactosa, también conocida como hipolactasia primaria, ha sido vista durante mucho tiempo como un problema clínico, casi como una enfermedad. Sin embargo, el principal genotipo asociado a la hipolactasia primaria, y su correspondiente mutación genética humana, son el factor más común a nivel mundial, lo que supone un amplio mercado de personas que podrían consumir más productos de origen lácteo, en línea con el valor nutritivo de la leche y sus derivados para la salud humana. En esta revisión de actualización bibliográfica se profundiza en los nuevos conceptos en cuanto al estudio de esta problemática y sus implicaciones tanto en la práctica clínica como en las industrias alimentarias y farmacéuticas. Uno de los aspectos más interesantes que abordan el estudio de la intolerancia a la lactosa es precisamente un cambio en la perspectiva y en la concepción de este problema, lo que determina una recomendación general de sopesar las propiedades nutritivas de la leche y derivados quitando importancia los riesgos que la intolerancia a la lactosa puedan suponer. Este nuevo enfoque, que ha surgido a partir de las evidencias científicas, estaría definiendo las próximas investigaciones y desarrollos industriales con vistas a consolidar la comprensión del papel de la leche como alimento funcional y la disponibilidad de alternativas que ayuden a mejorar los síntomas digestivos que puedan ser atribuidas a la lactosa, sin ignorar los beneficios de la leche.
