Le suivi moléculaire du taux de transcrit BCR-ABL par PCR quantitative est de plus en plus utilisé pour évaluer la réponse des patients au traitement. Cela est devenu particulièrement intéressant à l'ère de l'imatinib lorsque la maladie résiduelle descend en deçà du seuil de détection de la cytogénétique conventionnelle. L'objectif principale de notre travail est d'étudier l'évolution de la LMC par la quantification du transcrit moléculaire BCR/ABL durant la phase thérapeutique. Il s'agit d'une étude rétrospective, descriptive porté sur 30 patients suivis dans le service d'hématologie du CHU de Tizi Ouzou, à l'unité de consultation. Le suivi moléculaire a montré que 70% des patients ont eu une RMM à l'imatinib 400 mg et 20% des patients ayant eu un échec à l'imatinib 400 et 600 mg ont été substitués pas un ITK de deuxième génération avec une RMM observée à 100 %. La quantification du ratio BCR/Abl a montré que 96% des patients sous imatinib 400mg étaient en voie de guérison. En conclusion, l'optimisation du traitement et l'utilisation des différents ITK disponibles nécessitent dès le diagnostic la mise en place du monitoring biomoléculaire portant sur les transcrits BCR/Abl.
