La levothyroxine (LT4) est un dérivé lévogyre isomère naturel, composé de levothyroxine sodique et se présentant sous la forme de comprimé de 50 quadrisécables, très utilisée par les angloxasons. A poids égal, elle est deux fois plus active que la DL thyroxine. Cette dernière est un mélange racémique habituellement utilisé et présenté sous la forme de comprimé de 0,5 mg, de goutte de 2 mg/ml et d'ampoule de 1 mg injectable en intramusculaire. Un troisième dérivé, la D thyroxine, n'est pas utilisé en pratique ; il a une action métabolique d'ailleurs faible.La dose par kg de poids corporel chez nos patients se trouve entre celle proposée (1,72 + 0,36 microg/kg/j) et (2,08 + 0,58 microg/kg/j) ; les deux séries ayant un âge moyen équivalent à celui de notre série. En revanche, des patients plus âgés ont reçu les doses par poids corporel identique (1,86 + 0,08 microg/kg/j) chez le patient de plus de 60 ans ; d'autres auteurs ont proposé une dose plus faible (1,60 microg/kg/j) pour un âge moyen de 68 ans.Le sel iodé réduit de 40 à 95% la fréquence du goitre endémique quand il est utilisé sous forme de sel de table enrichi à l'iodure de potassium /Europe, Australie,Nouvelle-Zelande...
