L’allaitement maternel est l’alimentation infantile de référence mais reste peu pratiqué en France. Nous présentons son épidémiologie et ses bénéfices pour la mère et l’enfant, ses aspects pratiques. Nous étudions ensuite sa promotion en Basse-Normandie par les associations et les maternités, puis ses déterminants par 2 enquêtes auprès de femmes et d’hommes. L’allaitement apparaît comme une expérience humaine forte: la transmission familiale y est primordiale et l’engagement important; l’allaitement est un choix de couple. Son refus repose souvent sur des craintes liées à des idées reçues. Nos enquêtes révèlent le défaut d’information des parents, l’absence d’implication des maternités, le recours rare aux associations, la mauvaise formation des médecins généralistes. Nos propositions concernent notamment l’information systématique des futurs parents, la formation des personnels de maternité et des médecins généralistes, la place du médecin traitant et les échanges entre les différents accompagnants à l’allaitement. Des changements effectifs demandent une volonté politique qui fait encore défaut, malgré l’existence de dispositions internationales pour favoriser l'allaitement.
