Die mediale, intrakapsuHire Schenkelhalsfraktur ist die klassische "unsol ved fracture", wobei sich die Aspekte in der Betrachtungsweise des Pro blemkreises seit Nicolls Arbeit mit selbigem Titel -aus dem Jahre 1963 wesentlich verandert haben. Damals standen vor allem die Pseudarthrosen als Hauptkomplikationen im Mittelpunkt des Interesses. Dass die Heilung dieser Frakturen nach Osteosynthesen mit "dynamischen" Schraubensy stemen mit gutem funktionellen Ergebnis die Regel ist, wird heute niemand mehr bestreiten. Das gegenwartige Hauptproblem ist die avaskulare Hiiftkopfnekrose (AHN), sowohl nach funktioneller Behandlung von impaktierten, stabilen Briichen als auch nach Osteosynthese von dislozierten, instabilen Fraktu ren. Schon bei dieser Feststellung werfen sich die ersten Fragen zur Genese dieser typischen Komplikation auf. Nach den bisherigen Vorstellungen und anatomischen Studien ist eine Hiiftkopfnekrose nur bei dislozierten Frak turen mit Verletzung der EpiphysengefaBe zu erwarten. Nach konservativer Behandlung von stabilen Frakturen kommt es in 11- 18% der FaIle zu Hiiftkopfnekrosen. Was ist die Ursache dieser Nekrosen und warum entsteht nicht in jedem Fall bei dislozierten Frakturen mit Frakturver lauf durch den Eintritt der EpiphysengefaBe in den Oberschenkelkopf eine Nekrose? Gibt es Revaskularisationsphanomene und wie kann man sie dar stellen? Diese Fragen, ebenso wie die Frage nach der Rolle des Osteosynthese implantates fiir Stabilitat und Vaskularisation, sind neben der Perfusionssto rung durch das Hamarthros in der Kapsel in diesem Themenkomplex immer wieder Anlass fUr lebhafte Diskussionen, die am Ende meist ohne wissen schaftliche Absicherung mit weiteren offenen Fragen enden.
