Insulinresistenz ist durch eine verminderte zelluläre Reaktion auf Insulin gekennzeichnet, die hauptsächlich auf eine gestörte Glukoseaufnahme, eine verminderte Glykogensynthese und einen veränderten Glukosestoffwechsel zurückzuführen ist. Sie betrifft vor allem die Skelettmuskulatur, das Fettgewebe und die Leber und führt zu einer gestörten Glukose- und Lipidhomöostase. Die Skelettmuskulatur weist eine verminderte Glukoseverwertung auf, das Fettgewebe zeigt eine erhöhte Lipolyse und die Freisetzung von Entzündungsmediatoren, und die Leber zeigt eine gestörte Glukosesuppression mit erhöhter Lipidakkumulation und VLDL-Sekretion. Die Insulinresistenz beeinträchtigt auch die Endothelfunktion, indem sie die Bioverfügbarkeit von Stickstoffmonoxid verringert und durch eine selektive Beeinträchtigung des PI3K-Signalwegs pro-atherogene Signale fördert, was zu vaskulärer Dysfunktion und Atherosklerose beiträgt.
