A diálise peritoneal (DP) é um método de substituição renal cuja eficácia está comprovada e que constitui uma alternativa útil à hemodiálise e ao transplante renal no tratamento da insuficiência renal crónica terminal. É uma técnica de depuração extra-renal baseada no princípio das trocas, através de uma membrana peritoneal semipermeável, de solutos segundo um gradiente de concentração e de solvente segundo um gradiente de depressão. As duas modalidades terapêuticas são a diálise peritoneal ambulatória contínua (CAPD) e a diálise peritoneal automatizada (APD), com as suas variantes. As explorações funcionais do peritoneu e os critérios para uma diálise adequada permitem adaptar a modalidade de tratamento em função da função renal residual e do grau de permeabilidade da membrana peritoneal, bem como controlar a qualidade do tratamento a fim de reduzir a morbilidade. As complicações são dominadas pelas infecções peritoneais, o mau funcionamento do cateter e a perda de ultrafiltração, que podem exigir a transferência para a hemodiálise.
