La periodontitis es una enfermedad multifactorial para la que se proponen varios factores de riesgo y susceptibilidad en la historia natural de la periodontitis. Un factor de riesgo de periodontitis es un factor (ambiental, conductual o biológico) confirmado por secuencia temporal generalmente en estudios longitudinales. Su presencia aumenta directamente la probabilidad de que aparezca la enfermedad y la ausencia de un factor de riesgo reduce esta posibilidad. En un estudio longitudinal clásico sobre la historia natural de la periodontitis, Loe et al descubrieron que entre los individuos con una higiene bucal deficiente y sin acceso a atención odontológica, algunos desarrollaban la enfermedad a un ritmo rápido, mientras que otros experimentaban poca o ninguna enfermedad.
