Ziel der endodontischen Behandlung ist die Desinfektion des Pulpaspaltes und die anschließende Versiegelung dieses Raumes, um eine Rekontamination zu verhindern. Es wurden Erfolgsquoten von 47-97 % für nicht chirurgische/orthograde Wurzelkanalbehandlungen berichtet, wobei Misserfolge eher mit dem präoperativen Vorhandensein von periapikaler Röntgenstrahlenbildung, Wurzelfüllungen mit Hohlräumen, Wurzelfüllungen, die mehr als 2 mm vor dem röntgenologischen Apex liegen, und unbefriedigenden koronalen Restaurationen in Verbindung gebracht werden. Chirurgische Endodontie ist ein Begriff für chirurgische Eingriffe an den Zahnwurzeln und dem periapikalen Gewebe. Sie ist indiziert, wenn die konventionelle Endodontie versagt hat oder nicht durchführbar ist. Ziel der chirurgischen Endodontie ist die Wiederherstellung der Integrität des Stützgewebes eines Zahns oder von Zähnen mit chronischer pulpaler oder periapikaler Erkrankung, wenn die nicht-chirurgische Endodontie versagt hat und eine erneute Behandlung nicht möglich oder kontraindiziert ist. Die endodontische Chirurgie hat sich inzwischen zur endodontischen Mikrochirurgie entwickelt. Durch den Einsatz modernster Geräte, Instrumente und Materialien, die biologische Konzepte mit der klinischen Praxis in Einklang bringen, führt der mikrochirurgische Ansatz zu vorhersagbaren Ergebnissen bei der Heilung von Läsionen endodontischen Ursprungs.
