Auflage dieses Buchs hat die Onkologie eine Reihe von Wandlungen und Neuerungen erlebt, die auf die internistische Krebstherapie nicht ohne Auswirkung ge blieben sind. Insgesamt ist die Behandlung des Krebskranken anspruchsvolIer geworden, v. a. bedingt durch die Erweiterung interdisziplinarer The rapiekonzepte und die zunehmend nach Prognosefaktoren adaptierte Wahl der Therapie im EinzelfalI. Hinzugekommen sind aber auch neue Medikamente in Mono-oder Kombinationstherapien, neue Erkenntnisse iiber Pharmakokinetik und Pharmakodynamik von Hormonen und Zytostatika und ein alIgemein besseres Verstandnis der Biologie und Spontanprognose maligner Tu moreno Glaubte man vor einigen lahren noch, die intemistische Krebsthe rapie schrittweise vereinfachen und sie damit mehr und mehr in die freie Praxis iiberfiihren zu konnen, zeigt sich heute eher ein umgekehr ter Trend. Die Zunahme therapeutischer Moglichkeiten und deren ge zielte Anwendung und Anpassung an verschiedene Untergruppen maligner Tumoren ein und derselben Histologie macht die Indika tionsstelIung zur Systemtherapie zu einer Materie, die nur noch der wirklich Erfahrene beherrschen kann. Zudem lassen sich die theore tisch moglichen Therapieergebnisse praktisch nur erzielen, wenn die immer feiner herausgearbeiteten Therapieschemata wirklich genau befolgt und an die individuelIe Krankheits-und Toxizitatslage ange paBt werden. Nichtbefolgen solcher Richtlinien hat Z. B. vorzeitige Re sistenzentwicklung von Tumoren zur Folge, was bei potentielI heilba ren Tumoren fatal sein kann. Ahnliches gilt auch fiir die adjuvante Chemotherapie. Der internistische Onkologe am Zentrum hat in die ser Situation eine neue Verantwortung im Rahmen der konsiliarischen Beratung seiner nichtspezialisierten KolIegen in Krankenhaus und Praxis iibemommen.
