Fachhochschule Jena, Sprache: Deutsch, Abstract: Die Gesundheit ist das höchste Gut in einer leistungsorientierten Zeit. Sie ist die Voraussetzung für den Arbeitnehmer, seine Arbeitskraft auf dem Arbeitsmarkt zur Verfügung zu stellen. Jedoch ist die Krankheit ein Bestandteil von Gesundheit und in der ganzheitlichen Betrachtungsweise des Menschen ein Alarmsignal des Körpers. Es ist die Reaktion eines komplexen biochemischen Systems, welches adaptiv auf eine veränderte Umwelt reagiert. Eine zunehmende Überforderung oder auch Unfähigkeit dieses Systems lässt jedoch Krankheitsbilder auftreten, welche medizinische aber auch zu einem nicht unerheblichen Teil sozialpsychologische Intervention benötigen. Mit Einführung der Diagnosis Related Groups (DRG) wurde die Pflegeüberleitung (oder auch Überleitungspflege, Übergangspflege oder Brückenpflege genannt) eingerichtet. Sie soll den Prozess von stationärer nach ambulanter Verlegung administrativ begleiten und im interdisziplinären Kontext der Professionen und Gruppen von Arzt, Pflegeteam, Patient und Angehörige stattfinden. An dieser Stelle soll aber nur eine erste Formulierung der Übergangsbegleitung abgegeben sein. Frau Hecht wird diese präzisieren und am konkreten auf die spezifischen Anforderungen im Arbeitsfeld an der Schnittstelle zwischen ambulanten Pflegdienst und Krankenhaus für die Soziale Arbeit anpassen. Ein Fazit dieser Diplomarbeit, welche durch seine Zweiteilung einen größeren Rahmen erfassen soll, kann am Ende des 1. Teils nur innerhalb des Rahmens der Wissensansammlung und an der Prozessanalyse und Kompetenz-Performanz Betrachtung erfolgen.
