Der heute noch wenig verwandte Begriff der "antero-Iateralen Rotationsinstabilitat des obe- ren Sprunggelenkes" definiert exakt die Pathomechanik der fibularen Bandruptur am obe- ren Sprunggelenk als eine Instabilitat in 2 Ebenen, d. h. Talusvorschub, Taluskippung und pathologische Innenrotation des Talus bei Varusstress. Dieses Krankheitsbild, das erstmals 1916 von Mohring als Fall einer, habituellen Luxatio pedis" im deutschen Sprachraum be- kannt wurde, wird heute als eines der hiiufigsten Verletzungen im Freizeit- und Leistungs- sport beobachtet. Ftihrende Vertreter der Orthopadie und friihen Traumatologie wie Bohler und Watson- Jones empfahlen, nur die Versager der konservativen Behandlung zu operieren. So wurden bis heute liber 40 verschiedene, meist unphysiologische operative Methoden zur Behandlung der chronischen Instabilitat des oberen Sprunggelenkes bekannt. Erst in den sechziger Jah- ren setzte die Ara der primar operativen Behandlung bei fibularer Bandruptur ein, was seit Beginn der achtziger Jahre in der Literatur emeut kontrovers diskutiert wird. An unserer Klinik hat sich besonders Herr Zwipp mit den Bandverletzungen am Sprung- gelenk klinisch, experimentell und wissenschaftlich befa1k Die Ergebnisse seiner Untersu- chungen hat er in seiner Habilitationsschrift zusammengef t. Sie enthalt die gro te bisher publizierte Analyse der Weltliteratur von liber 1500 Fallen einer akuten/chronischen Insta- bilitat des oberen Sprunggelenkes. Neben der Erhebung von 5-Jahres-Ergebnissen gro er konsekutiver Kontrolluntersuchungen, welche die Vorteile der operativen Behandlung un- terstreichen, erganzen epidimeologische, atiologische, anatomische und histologische sowie pathomechanische Untersuchungen die Phanomenologie dieses Krankheitsbildes. Dadurch konnten differentialdiagnostische Techniken wie die gehaltene Aufnahme des Subtalargelen- kes und verfeinerte anatomisch-physiologische Operationsverfahren in die klinische Routi- ne Einzug halten.
